What this website asks for
Forms on this site are for adults and organizations: name, email, phone, and the message you choose to write. They ask for partnership, volunteer, referral, or general contact information. They do not ask for a minor’s story, case details, or documents.
What happens to a form right now
A submission is emailed to admin@cosmic-futures.org. This website does not keep a database of form messages. FormSubmit carries the email so it can reach that inbox. Do not send a young person’s story, case file, or contact details through these forms.
What a future policy must cover
Who can see contact records, how long they are kept, how someone asks for a correction or deletion, and which vendors, if any, touch the data. That policy will be adopted before the organization stores participant information.
